No one wants to be in a motor vehicle crash. Unfortunately, most of us will be in a crash at some point in our lives. Massachusetts law requires that any operator of a motor vehicle that is involved in a crash in Massachusetts must complete and submit a Massachusetts Motor Vehicle Operator crash report. Please read this paragraph for instructions on how to print the form. When you print the crash form print the first page only of the crash form and then place the printed page back into your printer to print the second page on the back side of the first page. If you are not sure how to do this, you should be able to consult the manual that came with your printer to get the instructions. You need to print (3) three copies of the form. One copy of the completed form goes to the police department of the town where you had the crash, one copy goes to your insurance company, and one copy goes to the Registry of Motor Vehicles. The address for the Registry of Motor Vehicles is printed right on the crash form in the upper left corner.
